Most probably it is CANCER OF STOMACH. (рак желудка)

We can see the bulb of duodenum.
There’s pouch on relief and on outline.
Conclusion:
ULCER OF DUODENUM AND BULB OF DUODENUM. (Язва двенадцатиперстной кишки и луковицы двенадцатиперстной кишки)

Bulb of duodenum is deformed.
We can see small particles on duodenal bulb.
Conclusion:
